Candidates must have safe handling of medication, 3 Day first aid trained. Candidates must also have NVQ Level 3 or above. • Provide technical and professional advice to the service and its customers • Work independently as part of a specialist team managing a caseload undertaking assessment of need, creating needs led plans to deliver evidence based interventions and undertake reviews with consideration for budgetary constraints and in accordance with legislation and council policies. • Provide ongoing advice and support to vulnerable clients and their carers including Key worker responsibility • Prepare, plan and facilitate implementation of the individual’s care plans, support plans, risk assessments and monitor the effectiveness of these plans and take any appropriate follow-up action in conjunction with the service user, carers and other professional as appropriate. • Work in partnership with others to ensure a consistent and planned care package is delivered. • Ensure that parents/carers, service users receive appropriate information and are aware of their rights under appropriate legislation so they can make informed decisions. • Promote self-advocacy and ensure at all times that the customer is central to the service. • Provide information, advice and guidance to customers and carers on the availability of local services and support available from voluntary and statutory agencies • Produce and maintain up to date accurate records to an appropriate standard as required by policy and good practice by using appropriate IT equipment and other methods • Develop strong working relationships with services, agencies and providers to contribute to effective multi-agency working and ensure that non-delivery by agencies is challenged appropriately.
