Job – Specialist Clinical Pharmacist Location – The Princess Grace Hospital, London Contract Type – Permanent, full time hours 37.5hrs Salary – Competitive (dependent on skills & experience). This position is equivalent to a Band 6-7 Job Description To provide a safe and patient-focused clinical pharmacy service to wards. This includes undertaking medicines reconciliation, assessing pharmaceutical needs, assessing and facilitating the use of patient’s own drugs, ensuring appropriate drug supply. Duties & Responsibilities • To participate in multi-disciplinary team work, helping to create and maintain effective working relationship and to liaise with the Lead Pharmacy Technician in all aspects of the department. • To independently provide a dispensing service to both in / out patients using the Meditech dispensing system under the supervision of a pharmacist. • To be able to provide patient medication counselling and to supply stock medications to outpatient clinics. • To ensure correct and prompt charging / invoicing for patients as required. • Maintain and control stock levels for both the pharmacy and wards. • To accurately check off deliveries against delivery notes, process claims for missing/incorrect items and returns. • To be involved in stock rotation, cycle counts and routine expiry date checks both within the pharmacy and on the wards. • To provide an efficient, accurate and economical ward top-up service according to hospital protocol and able to use the Omnicell® system. Skills and Experience • To participate in ward rounds in order to make pro-active interventions on drug related issues. • To communicate with the multidisciplinary healthcare team (doctors, nurses, dietitians, physiotherapists, occupational therapists, and ward managers), to intermediate care, general practice support and community pharmacists in the provision of medicine related information and assessment of patients treatment. • To participate in clinical and departmental audits, measure adherence to local and national guidelines and provide feedback to staff • To promote effective prescribing across the hospital following formulary, NICE, and any other relevant national or local guidance. • To support the education and training of other healthcare professionals, including medical staff, nursing staff and allied health professionals • To participate in weekend, late-duty and bank holiday rotas as required and to participate in the pharmacy on-call service. Skills and Experience • B. Pharm / M. Pharm / BSc (Pharmacy). • Registered with the GPhC. • Working towards a postgraduate Diploma in either Clinical Pharmacy or General Pharmacy Practice (or equivalent – MSc in Clinical Pharmacy). • Demonstrable experience in clinical pharmacy practice covering different specialities. • Member of the Royal Pharmaceutical Society is desirable.
