The aim is to contribute to the development of Multi-Disciplinary working within the Locality teams; the successful applicant will carry out prescribed care and Support as documented in the Multi-Disciplinary Team care plans, which will include Components of Social / Health and Rehabilitive Care. The role will be to support individuals in their own homes or other Community settings to maximise their independence and improve health well-being in a coordinated way. You will be supporting people to live at home by assisting and supervising individuals to carry out personal care and life skills, promoting and encouraging independence and confidence where possible as identified in the individual’s outcome focussed care plan. In addition you will be carrying out clinical interventions in accordance with the care plan, this will include reporting and recording results in accordance with local and organisational requirements. Examples could be taking blood, testing urine specimens, oral hygiene; physiological measurements (blood pressure, pulse and temperature). The post is based within established reablement teams, supporting individuals to regain their independence working with people over the age of 18 within their own homes, utilising external resources and ensure access to community networks and preventative (wellbeing) services. The post will be within a multi-agency team, which includes Nursing Staff, Physio Therapy, Occupational Therapy and Social Workers. Colleagues within other services include Community Equipment and Telecare Services and Voluntary Organisations. The Team delivers services 365 days of the year. We are therefore looking for individuals who has a flexible outlook and who would be willing to work unsocial hours in order to meet the needs of the individuals.
