On April 7, 2010, the U.S. Internal Revenue Service released Form W-11, Hiring Incentives to Restore Employment HIRE Act Employee Affidavit, for use by employers to confirm that a new employee qualifies for the special payroll tax exemption that applies for certain newly hired workers during 2010 pursuant to a provision of the Hiring Incentives to Restore Employment Act as enacted in March 2010.
