Moving pets

I appreciate the information, as I am planning to travel to both France and Belgium for an extended period due to my job, and I do not want to leave my two cats behind. I understand that certification may be required by France (source: http://quatrain.hubpages.com/hub/How-to-Travel-With-Your-Pet-to-France ). Do you have copies of the form France requires?
